KeystoneCare is seeking a dedicated, compassionate, and driven Registered Nurse (RN) to join our growing Home Health team on a full‑time basis! Primary Job Responsibilities:

  • Conduct comprehensive patient assessments to evaluate their healthcare needs.
  • Develop, implement, and update individualized care plans based on patient assessments and preferences.
  • Administer medications and treatments as prescribed by the physician.
  • Monitor and document patient progress and responses to treatment.
  • Educate patients and their families on health management, medication administration, and care plans.
  • Coordinate care with interdisciplinary team members to promote optimal health outcomes.
  • Ensure compliance with agency policies, state, and federal regulations.
  • Act as a patient advocate, promoting their rights and preferences in care.

Requirements

  • Current RN license in good standing to practice in the Commonwealth of Pennsylvania.
  • Graduate of an accredited nursing school (BSN preferred).
  • At least 1 year of nursing experience; experience in home health preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent interpersonal and communication skills.
  • Ability to work independently and manage time effectively.
  • Reliable transportation and valid Driver's License.
  • Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
  • Proof of Annual Influenza vaccination or an approved Medical or Religious exemption.
