Overview
Licensed Practical Nurse (LPN) – travel assignment in a long-term care setting located in Watertown, New York. Start date: 05/04/2026 for 13 weeks. Guaranteed 40 hours per week with 8-hour day shifts (8x5). Weekly pay range: $1,348–$1,391 per week. This travel role is based in a nursing home/long-term care environment, with RN supervision and a collaborative care team.

Responsibilities
- Provide direct, basic nursing care to residents under the supervision of a registered nurse
- Administer prescribed medications and document administration in PointClickCare EMAR per facility policy
- Monitor vital signs, assess for changes in condition, and escalate to the supervising RN as needed
- Assist residents with activities of daily living and support care planning for dementia, chronic diseases, and diabetes
- Perform routine nursing tasks such as wound care and adherence to infection control and safety protocols
- Collaborate with the interdisciplinary care team to update care plans and maintain accurate documentation

Requirements
- Active New York Licensed Practical Nurse license
- BLS (AHA) certification
- 1–3 years of LPN experience; SNF (skilled nursing facility) experience preferred, LTC experience acceptable
- Experience caring for residents with dementia, chronic diseases, and diabetes
- Proficiency with PointClickCare EMAR and ability to document accurately
- Ability to work 8-hour day shifts in a long-term care setting
